Coast Guard Releases Results of Investigation of Loss of S/S EL FARO

On October 1, 2015, the cargo vessel S/S EL FARO encountered the rapidly developing Hurricane Joaquin and sank resulting in the loss of all 33 members of the crew. The U.S. Coast Guard conducted a formal inquiry and released its formal report two years to the day after the catastrophic event on October 1, 2017. The Marine Board of Investigation found the owner of the vessel and captain failed to identify the risk of heavy weather, that the owner, master and officers were unaware of the vulnerabilities of the vessel to heavy weather among other things and made a host of recommendations which can be found on page 193-198 of the complete report (199 pages) which may be found following this link:https://media.defense.gov/2017/Oct/01/2001820187/1/1/0/FINAL%20PDF%20ROI%2024%20SEP%2017.pdf
